At Aabcor, Inc. our care managers offer a comprehensive range of support services to assist Medicaid Waiver customers and their families.

These services include:


• Maintaining regular telephonic contact with customers to ensure services align with their individual service plans.

• Addressing customer concerns, coordinating with HSP Rehabilitation Counselors, and providing ongoing support as needed.

• Monitoring service delivery and quality through regular wellness checks and care assessments to safeguard customer well-being.

• Connecting customers with appropriate providers and community-based resources to enhance their care and quality of life.

Support We Provide:

• Medicaid Assistance: Guidance with applications and renewals

• Ongoing Support: Providing empathy and understanding of customers needs and experiences

• Essential Services: Assist with referrals for medical equipment, transportation, financial services and more

• Brain Injury Education: Help customers and families better understand Brain Injuries (BI).

• Caregiver Support: Guidance and support to enhance care strategies.

• Advocacy: Ensuring access to services that benefit and support independence and well-being.

Want to Work With Us?

At this time, those interested in receiving BI Waiver services must go through the Illinois Department of Health and Human Services (HFS) and the Illinois Department of Human Services - Division of Rehabilitation Services (DHS-DRS) and go through the referral process.

Aabcor, Inc. does NOT provide enrollment into the BI waiver program.

If currently enrolled in the BI Waiver and want to work with us, contact your HSP Counselor to refer your case to Aabcor, Inc. for case management services.
