Ask a Care Manager offers quick consultations with an experienced care manager who can help you understand your options, identify potential concerns, and provide clear next steps. These consultations are designed to give families practical guidance and reassurance when they are facing important care decisions.
1) Quick Care Consultation (30 minutes)
A focused consultation designed to help you get immediate guidance about a care situation.

Ask A Care Manager Consultations
We offer two types of sessions; 30 minutes and 60 minute consultations:
2) Care Strategy Consultation (60 minutes)
For families who need more time to discuss a complex situation.
During this consultation we can:
✔ Review the care situation in greater detail
✔ Discuss medical, cognitive, or behavioral concerns
✔ Explore service options and care strategies
✔ Identify potential risks and planning needs
✔ Provide guidance for coordinating care among providers and family members
Families often leave these sessions with a clearer plan and practical next steps.
You share your concerns and explain the situation.
We ask questions to better understand the care needs.
We discuss possible risks, options, and resources.
You leave with practical guidance and recommended next steps.
If further guidance is needed, a comprehensive care management plan may be recommended either in person or via online video conference to create a detailed care plan.
