Aabcor, Inc., is a woman and minority-owned business, certified by Illinois’ Business Enterprise Program (BEP). We were established in 1997 as a private case management company. For more than 20 years, we have worked with over 5,000+ clients and understand the importance of providing individualized care.

We work closely with rehabilitation hospitals, health care facilities, physicians, psychologists, and other health care professionals across the Chicagoland area. Our experienced and skilled staff come from a variety of backgrounds that include medicine, nursing, mental health, and social work.

To make our mission possible:

Our Referral Intake and Community Liaisons work with rehabilitation hospitals, health care facilities, physicians, psychologists, neuropsychologists and other brain injury professionals. They provide immediate follow-up to referrals and ensure that case managers visit the survivor, and their families promptly after being discharged from facilities.  They also coordinate with other medical professionals to review survivors’ medical records and related documents in order to recommend a care plan that is tailored to the survivors needs.

Our Case Managers possess knowledge, skills, and experience necessary to provide quality services to individuals with multiple complex needs.  With baccalaureate and graduate degrees in social work, psychology, counseling and other related fields. Our case managers build and maintain a relationship with survivors and their families by acting as links to resources, services, and opportunities that contribute to the development and improvement of their lives.

Our Care Giver Support team is available to respond to problems and concerns as they arise and coordinate solutions quickly.  They also provide follow-up and on-going support to survivors and their families to ensure that their needs are being properly met. 

Our Human Resources, Information Systems, and Quality Improvement teams provide the underlying support to keeping our organization running seamlessly – from recruiting, and training qualified staff, to maintaining records and computer systems, and ensuring consistent improvement in the quality of our services.
