Job Summary

The Case Manager will play a critical role in providing comprehensive case management services to clients with complex health needs. This position involves working closely with clients, families, healthcare providers, and community resources to develop and implement individualized care plans. The ideal candidate will have a strong background in social work, psychology, or a related field and a passion for improving client outcomes.

Key Responsibilities

  • Conduct comprehensive assessments of clients' health, social, and emotional needs to develop tailored care plans.

  • Coordinate services with healthcare professionals, rehabilitation facilities, community organizations, and family members to ensure seamless care delivery.

  • Monitor clients' progress, reassess needs, and adjust care plans as necessary to support long-term well-being.

  • Serve as a primary point of contact for clients and families, providing ongoing support, education, and resources.

  • Advocate for clients to ensure they receive the appropriate services, benefits, and support from community programs and healthcare providers.

  • Document case management activities, maintain accurate client records, and adhere to all confidentiality and compliance requirements.

  • Participate in team meetings, training sessions, and professional development opportunities to maintain and improve case management skills.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Master's degree preferred).

  • Minimum of 2 years of experience in case management, healthcare, or social services.

  • Strong knowledge of Illinois waiver programs, healthcare systems, and community resources.

  • Excellent communication, organizational, and problem-solving skills.

  • Ability to work independently and collaboratively as part of a multi-disciplinary team.
