Brain Injury Program Overview and Description
Illinois Department of Human Services, Department of Rehabilitation Services

The Illinois Department of Human Services-Office of Rehabilitation Services and Aabcor have worked together to give families the assistance their loved one needs.

The Brain Injury Program is a home service program designed to support adults and children who have survived a traumatic brain injury or have acquired a brain injury resulting from strokes, infection, motor vehicle accidents or other related causes.

Aabcor will be able to accept referrals from the IDHS / DRS for the Brain Injury Home Services Program and evaluate applicants eligibility.

Program Eligibility

Applicants must meet the following definition of disability:
Medical diagnosis of acquired brain injury including: traumatic brain injury, infection (encephalitis, meningitis), anoxia, stroke, aneurysm, electrical injury, malignant neoplasm, and toxic encephalopathy.

The following conditions are not eligible for the Brain Injury Program:
Congenital disabilities such as cerebral palsy or epilepsy are not eligible diagnoses, nor are degenerative or neurologic disorders due to aging such as dementia, Parkinson's or Alzheimer's.

Age:
There is no age criterion associated with the Brain Injury program. Persons of any age may be served, if otherwise eligible.

Other Eligibility Criteria:
To be eligible for Brain Injury services, you must be:

a US Citizen or a legally admitted alien;
a legal resident of Illinois;
have a severe disability that will last at least 12 months or is permanent for life; and
be considered at imminent risk of institutionalization by in-home assessment which is called the Determination of Need (DON)


Case Management Function

In-Home Assessments:
Assessment of customer's eligibility and needs to get funds authorized for services. Assessments are completed after the referral is received.

In-Home Reassessments:
In-home reassessments to note changes in eligibility and needs for continuation of services and getting authorization of funds. This is completed every 6 months after the initial assessment.

Regular Phone Contact:
Regular phone contact with the customer/family to ensure proper delivery of services, motivation and problem solving. It is mandatory to make these calls a minimum of once a month.

Liaison:
Liaison with physicians, medical institutions, service providers & community agencies.

Maintaining Documents:
Maintaining documents as per the regulations for survey and audits by Department of Public Aid & Department of Rehabilitation Services.
